Position Overview:

We are seeking a Field RN Case Manager for a client in Bethlehem. This role involves providing direct patient care and managing cases within the community to improve healthcare outcomes. The position offers competitive salary, on-call stipends, and mileage reimbursement. Final hiring decisions are made by our client.

Responsibilities:

• Assess patient conditions and develop care plans.
• Maintain medical records and report patient status.
• Serve as the primary contact for patients, families, and healthcare providers.

• Coordinate with healthcare professionals for continuous care.

Requirements:

• Active RN license in Florida without restrictions.
• Minimum of 1 year supporting in-home patients.
• Proficiency in case management software, strong organizational and communication skills.
• Empathetic, detail-oriented, and able to work independently in the field.
