This full-time, day-shift role runs Monday through Friday, 8:00 AM to 4:00 PM, with a focus on in-home skilled nursing.
The nurse will conduct home visits, develop care plans, educate patients and caregivers, collaborate with a multidisciplinary team, and document care activities to ensure quality outcomes.
