Full job description

Our Home Health agency is seeking an experienced and dedicated Registered Nurse (RN) to perform OASIS Start of Care (SOC), Recertifications, and skilled nursing visits for homebound patients. This role also includes case management responsibilities , where the RN will oversee and coordinate care for an assigned patient caseload.

The ideal candidate is clinically strong, organized, compassionate, and able to work independently while maintaining high standards of patient care and compliance.

After 90 days of successful performance , the RN will have the opportunity to transition into a full‑time position with benefits .

Key Responsibilities

• Complete OASIS Start of Care (SOC), Recertifications, and Resumptions of Care
• Perform skilled nursing visits and ongoing patient assessments
• Manage and coordinate care for assigned patients (Case Manager role)
• Monitor patient condition and identify changes requiring intervention
• Communicate effectively with physicians regarding patient status and orders
• Develop and implement individualized care plans
• Perform clinical procedures (vitals, injections, wound care, etc.)
• Administer medications and treatments per physician orders
• Provide wound care, including wound VAC management
• Educate patients and families on disease management, prevention, and healthy habits
• Ensure accurate and timely documentation (OASIS and visit notes)
• Maintain compliance with Medicaid & Medicare, agency, and regulatory guidelines
• Supervise and support LVNs and home health aides
• Respond appropriately to urgent patient needs and emergencies
• Maintain a safe and clean patient care environment

Qualifications

• Active Registered Nurse (RN) license – Indiana
• Home Health experience REQUIRED
• Strong experience with OASIS documentation (SOC & Recerts)
• Knowledge of wound care and wound VAC preferred
• Excellent clinical assessment and critical thinking skills
• Strong organizational and time management skills
• Ability to work independently in the field
• Excellent communication and interpersonal skills
• Compassionate, reliable, and detail-oriented

Compensation

• OASIS SOC: $35.00 per Hour
• Follow‑Up Visits: higher for complex/VAC cases
• Other Skilled Visits: (amount not specified)

Higher compensation available for highly experienced RNs.
