RN Case Manager: Holistic Care & Care Coordination

Jobtailor seeks an experienced Registered Nurse to conduct face-to-face member visits and evaluate benefit plan eligibility, guiding transitions to Aetna programs as needed.

You will apply holistic clinical judgment, review prior claims, and collaborate with supervisors to address barriers, while traveling 50% or more within the assigned region.

You must have a WV license (or multi-state privilege) and at least 5 years of clinical practice, with proficient MS Office skills for documentation.
