Responsibilities
- Coordinate patient care plans and ensure timely case management progress.
- Assess patient needs, develop evidence-based care plans, and monitor outcomes.
- Track and report case milestones with clear metrics and targets.
- Maintain accurate, timely documentation in electronic health records.
- Collaborate with multidisciplinary teams to optimize patient care.
- Navigate reimbursement processes and verify authorizations for services.
- Identify high-potential cases and escalate risks to ensure proactive intervention.
- Ensure strict confidentiality and compliance with regulatory standards.
- Educate patients and families on care plans, resources, and follow-up.
- Engage in continuous learning to advance clinical skills and case management expertise.
- Possess strong pipeline mindset to forecast workload and outcomes by territory.
Experience/Qualifications
- RN licensure with current state license and practice privileges.
- Experience in case management or care coordination within healthcare settings.
- Proficiency in electronic health records (EHR) documentation and charting.
- Strong knowledge of care plan development and evidence-based interventions.
- Knowledge of healthcare privacy regulations (HIPAA) and data confidentiality.
- Ability to navigate authorizations and reimbursement processes with insurers.
- Excellent communication skills for patient education and multidisciplinary collaboration.
- Data-driven mindset with ability to track milestones, targets, and territory forecasts.
- Strong organizational skills to manage multiple cases and high activity visibility.
