RN Case Manager: Geriatric Care & Care Coordination

TruHealth is seeking an RN Case Manager to lead care coordination for Medicare Advantage members. You will visit patients in person and by phone, review claims, and collaborate with the health care team to plan, implement and evaluate patient care.

You must be a self-starter and team player with strong communication skills, capable of managing multiple priorities with minimal supervision and adapting quickly to change.
