RN Case Manager: Geriatric Care & Care Coordination

TruHealth in the United States (Georgia) is seeking an RN Case Manager to manage and support case management strategies for our Medicare Advantage plan. You will visit patients in person or via telecommunication to ensure proper nursing care, consult with physicians to correct errors, and collaborate with health care teams to assess and plan patient care.
