Stamford Health is hiring a full-time Nurse Case Manager to join our Case Management Team. The role involves coordinating patient care within the continuum of care among the interdisciplinary team, ensuring quality and cost‑effective care, and acting as an advocate for patients’ health care needs.

Responsibilities
- Coordinate the care delivered to an assigned group of patients.
- Assess, plan, and evaluate outcomes of care in collaboration with the health care team.
- Guide and direct care provided by nurses and other discipline members to ensure quality and cost‑effectiveness.
- Direct the plan of care within the continuum, incorporating patient/family input.
- Ensure compliance with utilization requirements, Medicare and Medicaid guidelines, and federal, state, and local regulations.
- Maintain ongoing communication with physicians, referral sources, caregivers, patients, and families.
- Initiate assessments, identify alternatives, and select appropriate action within scope of practice.
- Act as advocate for patient needs and identify barriers to care access.
- Participate in revenue processes including appeal/denial processes.
- Maintain professional relationships with referral sources and community resources.
- Serve as a resource for healthcare team members along the continuum of care.
Qualifications
- Graduate from an accredited School of Nursing.
- Valid license as Registered Nurse in Connecticut.
- Bachelor’s degree as Registered Nurse licensed in Connecticut is required.
- Strong clinical experience in acute care; preferred additional experience in home care, long‑term care, and case management.
- Professional certification in case management or a clinical specialty is highly desirable.
- Prior experience in case management, utilization management, or discharge planning; experience with multi‑aged and multicultural populations preferred.
- Minimum two years of acute care experience in a hospital setting.
