The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Essential Job Functions / Responsibilities
Patient Care
- Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
- Provide a complete physical assessment and history of current and previous illnesses.
- Utilize all elements of the nursing process to provide professional nursing care.
- Assess and evaluate patient status by writing and initiating plan of care, regularly re‑evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Use health assessment data to determine nursing diagnosis and develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
- Include the patient and family in the planning process.
- Initiate appropriate preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsel patients and families in meeting nursing and related needs.
- Provide health care instructions to the patient as appropriate per assessment and plan.
- Assist the patient with activities of daily living and facilitate the patient’s efforts toward self‑sufficiency and optional comfort care.
- Act as case manager when assigned by the clinical supervisor and coordinate patient care for assigned caseloads.
Communication
- Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given.
- Record pain/symptom management changes/outcomes as appropriate.
- Communicate with the physician regarding the patient’s needs, report changes in the patient’s condition, and obtain/receive physicians’ orders as required.
- Communicate with community health related persons to coordinate the care plan.
- Teach patients and family/caregivers self‑care techniques as appropriate.
- Provide medication, diet, and other instructions as ordered by the physician and recognize and utilize opportunities for health counseling with patients and families/caregivers.
- Work in concert with the interdisciplinary group and maintain a safe environment for the patient.
- Assist patients and families/caregivers and other team members in providing continuity of care.
- Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
- Attend interdisciplinary group meetings.
Additional Duties
- Participate in on‑call duties as defined by the on‑call policy.
- Ensure arrangements for equipment and other necessary items and services are available.
- Supervise ancillary personnel and delegate responsibilities when required.
- Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in‑service classes.
- Fulfill the obligation of requested and/or accepted case assignments.
- Actively participate in quality assessment performance improvement teams and activities.
Position Qualifications
- Graduate of a National League for Nursing accredited school of nursing.
- Minimum of two years of experience, at least one of which is in the area of public health, home care, or hospice nursing (preferred).
- Management experience not required; responsible for supervising hospice aides.
- Registered nurse with current licensure to practice professional nursing in the state.
- Demonstrates excellent observation, problem‑solving, verbal and written communication skills per competency checklist.
- Shows ability to organize and prioritize workload independently.
- Physical requirements: prolonged or considerable walking or standing; able to lift, position, and/or transfer patients; lift supplies and equipment; reach, stoop, bend, kneel, and/or crouch; visual acuity and hearing to perform required nursing skills.
- Maintains a current CPR certification.
- Must be a licensed driver with a car insured in accordance with state or organization requirements and in good working order.
- Self‑directed and able to work with minimal supervision.
