RN Case Manager - Flexible Schedule & Growth

Mercy Health St. Vincent Medical Center is seeking a qualified nurse to manage patient transitions of care from hospital to post-acute settings. The role requires ensuring payer approvals and accurate documentation that supports patient status, level of care, and DRG coding.

Current Ohio RN license is required with BSN preferred or to be obtained within 3 years. Candidates should have strong case management skills, DRG knowledge, and be able to coordinate with physicians, patients, families and
