What You'll Do

• Collaborate with the healthcare team to enhance patient care services.
• Continuously improve the patient experience while ensuring high-quality care.
• Evaluate, predict, and facilitate the trajectory of patient care.
• Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
• Participate in an in-depth orientation and onboarding program.
• Engage in the "Novice to Expert" program for professional development.
• Utilize resources from case management specialists and strong leadership support.

Education

ADN required, BSN strongly preferred

Experience

RN Case Manager experience required; Minimum 2 years Med/Surg experience required; Acute hospital experience preferred

Licenses & Certifications

Current RN License in Massachusetts; Case Manager certificate preferred
