Job Description
- Perform comprehensive assessments of high-risk members and develop individualized care plans.
- Collaborate with primary care providers to implement targeted interventions and ensure appropriate care levels.
- Identify and continuously reassess cost-effective and suitable levels of care for members.
- Coordinate non-medical support such as housing and transportation to promote treatment adherence.
- Manage care transitions by coordinating with physicians, social workers, discharge planners, and claims professionals.
- Engage specialty resources as necessary to optimize health outcomes.
- Document clinical, functional, and fiscal outcomes during treatment management.
Critical Information
- Monday - Friday, 8:00 AM - 5:00 PM, on-call for PRN every 5th week.
- On-site position in Elizabethtown, NC.
- Compliance and submission requirements applicable.
Education/Licenses Needed
- Associates or Bachelor’s Degree in Nursing or related field.
- Current, unrestricted state Registered Nurse license.
- Case Management certification.
