POSITION SUMMARY

RN Case Manager

POSITION DUTIES

Provides Case management service for both inpatient and observation patients assigned. Performs a comprehensive assessment of psychosocial, medical and discharge needs of patients/family, coordinates the plan of care and drives the discharge plan by collaborating with the multidisciplinary team. Evaluates progression of care using InterQual throughout the episode of care.

MINIMUM REQUIRED QUALIFICATIONS

  • TX/compact RN license
  • BLS
  • 2 years case management experience in an acute hospital setting
  • Acute hospital experience during the past year with no more than a 90-day break

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

8:00am-4:30pm with rotating weekends
