About This Role

As an RN Case Manager, you will provide high-quality nursing care to patients in their homes, supporting the East Team in various locations. This role is ideal for those who excel in patient assessments, complex nursing treatments, and care coordination.

• Pay: $69,680 - $111,488 annually
• Location: Independence, OH
• Facility: Cleveland Clinic Fairview Hospital
• Schedule: Full-time, Day shift

What You'll Do

• Provide direct nursing care to patients in their homes.
• Perform accurate and complete admission assessments and reassessments.
• Assess patients for safety risks, abuse, and neglect.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Assess, document, prioritize, and manage patients’ pain.
• Complete documentation and make calls after patient visits.
• Attend meetings and coordinate patient care.
• Arrange visit schedules for patients.

Requirements

• Education:
  • Graduate from an accredited school of Professional Nursing
  • Bachelor’s of Science in Nursing (BSN) preferred
• Experience:
  • One year of current experience as an RN
  • One year of experience in an acute Medical/Surgical or Home Care setting
• Licenses & Certifications:
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • Valid Ohio Driver's License upon hire
  • Proof of automobile insurance coverage upon hire
