RN Case Manager: Discharge & Care Coordination

Houston Methodist is seeking a Case Manager (RN) to plan case management, care transitions, and discharge planning for a targeted patient population. The CM works with physicians and the care team to ensure compassionate, efficient, quality care and desired outcomes.
