RN Case Manager — Discharge & Care Coordination Expert

Advocate Health is seeking an experienced Case Manager (RN) to identify patients who will benefit from discharge planning and to collaborate with a multidisciplinary team to optimize outcomes. The role requires a BSN with MSN preferred and a current RN license in the applicable state.

Five years of clinical experience and case management familiarity are expected. You will assess patient needs, develop and implement a collaborative Plan of Care, and coordinate communication with payors and
