The RN Case Manager is responsible for creating, implementing, and evaluating care plans for patients and their families across the health continuum. This role is ideal for those who excel in collaboration and advocacy for patients and caregivers.

What You'll Do
- Contribute to or complete comprehensive assessments of patients and their families/caregivers.
- Effectively identify barriers and analyze situations for the assigned patient population.
- Coordinate clinical care with the patient, family, provider(s), and interdisciplinary team members.
- Advocate for patients and families through effective communication with providers and team members.
- Leverage understanding of industry standards to provide applicable education to patients and families.
- Actively seek professional development opportunities through certification and mentoring.
- Adhere to the policies, procedures, rules, regulations, and requirements of clinical settings.
Requirements
- Education: Bachelor's Degree in Nursing required, or Bachelor's Degree and Nursing Diploma, or 6 years of relevant experience in lieu of a degree.
- Experience: 3+ years in a clinical nursing role.
- Licenses & Certifications: Current State of PA RN licensure or current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC). CPR certification required. Professional Certification within 5 years of start date preferred.
