RN Case Manager: Day-Shift Care Coordinator — Sign-On Bonus

Good Samaritan Loveland is seeking a professional RN to integrate health care for clients from pre-admission to post-discharge. This role coordinates care with an interdisciplinary team to improve patient flow and outcomes, linking clinical, financial and utilization data in a steady, goal-driven environment.

The schedule is Monday through Friday with some flexibility on which 4 of 5 days you are scheduled, primarily day hours with weekend requirements.
