Responsible for creating, implementing, and monitoring treatment plans, the full-time RN Case Manager - Compact State Licensed will work remotely to assist patients in meeting their healthcare goals through assessment, care coordination, and collaboration with healthcare providers. Key responsibilities Assess member health status and coordinate care needs, including inpatient review and discharge planning Develop and implement individualized treatment plans while monitoring health status and adjusting care as needed Identify and coordinate services and resources to promote optimal value and cost-effectiveness for clients Required qualifications 5+ years of related work experience in clinical nursing and patient assessment Bachelor's degree or equivalent related work experience Graduate of an accredited school of nursing RN - Registered Nurse - State Licensure with Compact State Licensure in good standing Knowledge of medical terminology and cost containment practices in healthcare
