Job Title: RN Case Manager

The Nurse Manager provides clinical, operational, and strategic leadership for nursing services at The Ohio State University Total Health Care Center (THCC), a Federally Qualified Health Center. This role balances direct patient care and care coordination responsibilities with management oversight to ensure delivery of high-quality, patient-centered care consistent with the Patient-Centered Medical Home model. The Nurse Manager leads nursing staff, oversees care coordination activities, promotes population health initiatives, drives quality improvement efforts, ensures regulatory compliance, and supports achievement of organizational goals related to access, quality, patient experience, and health outcomes. As THCC continues to grow, the Nurse Manager will play a critical role in expanding nursing and care coordination services across multiple clinical sites.
Minimum Qualifications
Bachelor's Degree in Nursing (BSN).
Current Ohio Registered Nurse (RN) licensure.
3 years of relevant experience required.
Preferred Qualifications
4-8 years of relevant experience preferred.
Experience in ambulatory care, care coordination, population health, chronic disease management, case management, PCMH, or FQHC settings preferred.
Master's Degree in Nursing, Healthcare Administration, Business, or a related field preferred.
