Qualifications
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current state license as an RN, current driver’s license.
- Experience: One year of RN experience in a clinical care setting required; home‑health experience preferred.
- Knowledge and Skills: Nursing skills as generally accepted standards; good interpersonal skills; CPR certification; ability to transport patients; ability to work flexible schedule and local travel.
- Transportation: Reliable transportation and valid driver’s license and auto insurance.
Responsibilities
- Administer skilled nursing care in patients’ homes for clients of all ages.
- Coordinate care with the interdisciplinary team, patients, families, and referring agencies.
- Complete initial and ongoing assessments, including OASIS or other appropriate tools.
- Re‑evaluate patients’ nursing needs and outcomes, updating the plan of care as necessary.
- Implement preventive and rehabilitative nursing procedures and initiate referrals to other services.
- Prepare clinical and progress notes and complete documentation in a timely manner.
- Supervise, teach, and provide clinical direction to home‑health aides, LPNs/LVNs, and paraprofessionals.
- Perform on‑call duties and provide care to patients and families as assigned.
- Communicate changes in patients’ condition to physicians and other team members.
- Ensure compliance with third‑party payor requirements and maintain adherence to infection control measures.
Essential Functions
- Coordinate and oversee patient care plans, discharge planning, and specialized nursing services.
- Maintain documentation and monitor compliance with regulations and payor requirements.
- Demonstrate professional growth through in‑service programs and competency improvement.
- Promote the agency’s philosophy and administrative policies.
