• Assessment of discharge needs, Social determinants of health, and discharge planning/referral for Acute Medical and Surgical patients.

MINIMUM REQUIRED QUALIFICATIONS

• OH/compact RN license
• BLS
• 3+ years Acute Care Case Management experience

LENGTH OF ASSIGNMENT

Days, M-F, 730a-4p; No call, no holidays, no weekends
