RN Case Manager: Care Transitions & Advocacy

Suburban Hospital, part of Johns Hopkins Medicine, is seeking a Case Manager to coordinate patient-centered care from pre-admission through post-discharge in collaboration with patients, families and the care team.

The role requires a BSN and an active RN license, with a minimum of two years clinical nursing experience; one year in case management or discharge planning is preferred. CPR certification is required.
