RN Case Manager – Care Coordination (Mon-Fri, Rotating Weekends)

UAB Medicine’s UAB St. Vincent’s in Birmingham, AL is seeking a Registered Nurse to coordinate the interdisciplinary plan of care from admission to discharge. You will work a schedule of 8:00 a.m.

to 4:30 p.m., Monday through Friday, with weekend and holiday rotation as required. Responsibilities include creating a care plan across the continuum, monitoring medical necessity, advocating for resources, and maintaining dialogue with the care transition team to ensure proper post‑discharge services
