RN Case Manager — Care Coordination Leader (PRN)

Moffitt Cancer Center is seeking a Case Manager PRN to join our multidisciplinary care team in Tampa, FL. The role focuses on coordinating patient care, advocating for patients, and ensuring appropriate utilization of health care resources across the continuum of care.

The Case Manager will collaborate with the health care team, perform risk assessments, and participate in discharge planning. Some weekend/holiday coverage may be required as part of the rotation.
