RN Case Manager: Care Coordination & Transitions

Ascension St. John's Medical Center in Tulsa seeks a seasoned Case Manager RN to coordinate patient care, validate medical necessity, advocate for benefits, and facilitate safe transitions to reduce readmissions.

You will partner with care teams to optimize discharge planning, protect revenue by avoiding denials, and ensure compliance with InterQual and payer rules.

Requires RN license, 5+ years RN, 3+ years case management; CCMC preferred; full-time, Mon-Fri, 8am-4:30pm.
