RN Case Manager: Care Coordination & Patient Advocacy

Fallon Health in Worcester, Massachusetts is seeking an RN Nurse Case Manager responsible for care coordination and management. The NCM will work with members to ensure they receive the necessary support and services as outlined in their care plans.

This role requires strong communication skills, an active RN license in Massachusetts, and at least 1 year of experience in clinical RN settings with complex needs. Candidates who are certified or willing to obtain certification in Case Management are preferred.
