RN Case Manager: Care Coordination & Discharge

UnitedHealth Group in Bangor, ME seeks a Case Manager RN to coordinate patient-centered care across the continuum and develop safe discharge plans with the care team. You will optimize LOS, patient experience, and reimbursement while ensuring smooth transitions and timely interventions.

Eligible candidates hold an unrestricted RN license, 3+ years in acute care, and are proficient with Windows-based systems; BSN and CCM/ACM are preferred.
