RN Case Manager

POSITION SUMMARY – RN Case Manager

Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

Minimum Required Qualifications –

• FL/compact RN license
• BLS
• 2 years recent case management experience in an inpatient acute hospital setting

Preferred Qualifications –

• 5 years recent case management experience in an inpatient acute hospital setting
• Care Management certification (ACM or CCM)

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

11am-11:30pm & 1 additional 8hr shift 11am-7:30pm, every other week, required for 4hrs of OT
