RN Case Manager

CarePartners, a mission-driven health insurance company, is seeking a Registered Nurse Case Manager for our Home Health team within Healthcare & Medical Services. In this role, you will coordinate and manage home-based care for members, conduct assessments, develop and monitor care plans, and collaborate with providers to ensure safe, effective transitions of care. You’ll advocate for patient needs, support disease and medication management, and promote preventive care.

We value compassion, teamwork, and community health improvement. RN licensure and home health or case management experience required; managed care experience preferred.
