RN Case Manager

Job Details: Position – RN Case Manager. Location – Appleton, WI 54911. Employment – Full Time (Monday‑Friday 8am‑5pm). Salary – $88,000–$97,000 per year.

About the Role

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver attentive one‑on‑one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician‑ordered medications and treatments with skill and empathy.
• Educate patients and their families on care techniques and symptom management.
• Track and communicate changes in patient condition, ensuring timely and effective responses.
• Keep detailed and up‑to‑date clinical notes to reflect patient progress and quality of care provided.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
• Contribute to other supportive duties as part of a compassionate, mission‑driven team.

Qualifications

• A heart for hospice care and a commitment to exceptional service.
• Valid Registered Nurse license in good standing in the state of employment.
• Valid driver’s license and reliable transportation.
• Current CPR certification.
• Strong observational and communication skills—both written and verbal.
• Flexible, team‑oriented attitude and willingness to support additional duties as needed.
