Job Description

Seeking an experienced Acute Care Case Manager to join our Case Management team. The ideal candidate will demonstrate adaptability and proficiency in utilizing Midas and Meditech systems for patient reviews and documentation. This role requires a focus on enhancing patient experience while managing a case load effectively post-orientation.

Responsibilities

• Manage a patient load of approximately 1:30-1:35 ratios.
• Ensure effective case management processes are followed for optimal patient outcomes.
• Collaborate with interdisciplinary teams to provide comprehensive care for patients.

Required Experience / Certifications / Licensure

• Previous travel experience in Acute Care Case Management.
• Proficiency in documentation systems: Midas and Meditech.
