Jobtailor is seeking a registered nurse to support patients through education, disease management, and health coaching across multiple lines. You will conduct behavioral or clinical assessments, coordinate coaching plans, and coordinate with providers to facilitate referrals.
Experience with chronic diseases, home health, and case management is preferred; bilingual in Spanish is a plus.
