RN Care Transition & Discharge Navigator

Wheeling Hospital in West Virginia is seeking a Registered Nurse to oversee targeted patient transitions and discharge planning. You will manage the process for designated caseload, collaborate with the multidisciplinary team, and ensure the plan of care is patient-centered, high quality, efficient, and cost effective.

This role requires an active RN license, 3 years of clinical experience, and, preferably, a BSN.
