RN Care Navigator

To support kidney health management, the fully remote RN Care Navigator will manage a caseload of patients, facilitating care coordination, conducting health screenings, and collaborating with healthcare providers to ensure quality outcomes. Key responsibilities Handle inbound and outbound calls to provide exceptional service to members Educate patients on kidney health, dialysis, and transplant options while engaging them in HealthMap's Kidney Health Program Coordinate care by acting as a liaison between patients, their support networks, and healthcare providers Required qualifications Active, unrestricted RN license Bachelor's degree or 6+ years of RN experience including 3+ years in case management CCM certification preferred Three years of experience in case management preferred Experience with nephrology or cardiology patients preferred
