RN Care Navigator

Managing a caseload of patients, the full-time RN Care Navigator will coordinate kidney health management, facilitate care delivery, and promote high-quality outcomes while working remotely. Key responsibilities Handle inbound and outbound calls, providing exceptional service and education on kidney health and treatment options Engage members in HealthMap's Kidney Health Program and follow up based on complexity and policy guidelines Collaborate with healthcare providers and community resources to coordinate care and resolve patient concerns Required qualifications Active, unrestricted RN license required Bachelor's degree required; 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree CCM certification preferred Three (3) years of experience in case management preferred Experience in a dialysis center or transplant center preferred
