RN Care Navigator | Hospital Case Management

University Health System seeks an experienced RN for a case management role responsible for screening patients at the front door to determine acute or observation bed placement. The position coordinates diversions to appropriate care settings and serves as a liaison to medical staff, nursing and families.

Texas RN licensure is required with BSN preferred and a preference for national certification. Three years of hospital experience and completion of a Clinical Documentation Improvement course
