RN Care Navigator: Discharge & Continuity of Care

Ochsner Health System in Louisiana is seeking a discharge planning RN to act as liaison among patients, families, and the multidisciplinary care team. You will coordinate post-acute care, discuss alternatives, and help develop and implement the patient’s care plan at discharge.

This role emphasizes communication, coordination with physicians, and minimizing readmissions, with the ability to travel between facilities as needed.
