RN Care Manager Zeeland

Scope of Work

Plans and prioritizes care for individuals and populations, focusing on strategies that promote optimal health within populations.

Demonstrates expertise and current knowledge in nursing care and manages a caseload of patients of varying complexity, seeking to improve patient, family, and health systems/community outcomes through educational concepts and preventive care in a managed care environment.

Assesses internal and external referrals to identify patient and significant others’ needs, intensity level, insurance benefits, and other resources.

Develops a plan of care and makes recommendations to PCPs, specialists, and other health‑care team members regarding care management strategies, identifying ways to maximize continuity of care across the continuum.

Assesses educational needs of patients, families, and team members and implements appropriate teaching strategies or makes referrals.

Communicates and collaborates with patients, significant others, providers, and payers to coordinate services that improve access to appropriate care across the continuum and promote optimal health in a cost‑effective manner.

Measures patient care outcomes, interprets reports, and analyzes data trends for groups of patients.

Documents patient data, plan, interventions, and outcomes according to department guidelines.

Qualifications – Required

• Bachelor's Degree or equivalent in nursing or a health‑care related field (all departments except Corewell Health Ludington).
• BSN must be obtained within 7 years for those who start in this RN role on or after January 1, 2017 (Corewell Health Ludington Hospital).
• 2 years of relevant clinical nursing and/or case management, managed care, or related field experience.
• 5 years of relevant clinical nursing and/or case management, managed care, or related field experience.

Qualifications – Preferred

• Registered Nurse (RN) – State of Michigan.
• Case Manager, Certified (CCM) – CCMC Commission for Case Manager Certification.
• Basic Life Support (BLS) – AHA American Heart Association or ARC American Red Cross.

Employment Details

Full time, Day shift (8:00 – 5:00, Mon – Fri), 40 hours per week.
