RN Care Manager - Weekend Evening Shifts (30h)

Boston Medical Center is seeking an RN Care Manager to coordinate care in the Emergency Department. The role focuses on assessing needs, planning interventions, and coordinating services to achieve quality, cost-effective outcomes. The position supports the care management process and accommodates a weekend/evening schedule.

The ideal candidate holds a nursing degree and MA RN license, with 3–5 years of clinical experience including case management. CCM certification is preferred.
