RN Care Manager - Senior Care Transitions (Remote)

ArchWell Health is seeking a Nurse Care Manager to coordinate and evaluate care for patients with acute and chronic conditions across the care continuum. You will develop comprehensive care plans with the care team and execute them to meet performance standards, regulatory compliance, and high-quality patient care.

You will work closely with hospital case managers, PTs, social workers, patients, and families to ensure safe transitions of care, discharge planning, and effective home care
