The Care Manager (RN) will:
- Provide case management to members, including medical, social, psychological, and physical.
- Develop, implement, and monitor the care plan to help meet the member's needs.

Qualifications:
- 2+ years of Case Management and/or Home Care experience
- Graduate of an accredited School of Nursing
- NYS Registered Nurse (RN) license
