Socket.dev is seeking a Case Manager RN to coordinate and transition patients through the continuum of care. You will influence length of stay, resource utilization, and readmission prevention by collaborating with physicians, nursing staff, Home Care, PCP offices, and other resources.
The role requires state RN licensure, a BSN or current nursing degree, at least three years in acute hospital care, and ACM/ CCM certification with active continuing education.
