RN Care Manager: Integrated Care & Coordination

Molina Healthcare is seeking a registered nurse to provide care management and coordination across the continuum. You will collaborate with physicians, caregivers, and the care team to ensure member progress and quality outcomes. Some travel may be required depending on contracts and state regulations.

Responsibilities include assessments, care plan development, and monitoring, with emphasis on communication, data entry, and coordination across behavioral health, LTSS, and community resources.
