We’re seeking a dedicated and independent Home Health Registered Nurse (RN) to join a patient-centered healthcare team serving communities across California, Hawaii, Oregon, and Washington.
What You’ll Do:
- Conduct comprehensive patient assessments and develop individualized care plans
- Provide skilled nursing care and ongoing evaluations in patients’ homes
- Educate patients and caregivers on treatment plans, medications, and preventive care
- Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care
- Maintain accurate and timely clinical documentation in compliance with agency standards
- Monitor patient progress and adjust care plans as needed
- Participate in on-call rotations as assigned
What You’ll Bring:
- Active Registered Nurse (RN) license in the state of practice
- CPR/BLS certification required
- Valid driver’s license, reliable transportation, and current automobile insurance required
- Home health or acute care nursing experience preferred
- Strong clinical judgment, communication, and patient education skills
- Ability to work independently while managing a patient caseload effectively
