RN, Care Manager – Home Health (Contract)

Location: California 95422
Pay: $2,351 – $2,635/week
Schedule: Day Shift, 8-hour shifts (40 hrs/week)
Coverage Area: Lake County, CA (mileage reimbursed)

Position Summary & Responsibilities

Manage and coordinate home health patient care plans

Collaborate with care team and ensure continuity of care

Utilize OASIS and HCHB for documentation and compliance

Requirements

Active RN license (CA) – Required

Home Health experience – Required

HCHB software experience – Required

OASIS experience – Required

BLS (AHA, HS-BLS, or RQI) – Required

BSN – Preferred

Additional Information

Travel required within Lake County (mileage reimbursed)

Must meet vaccination requirements (exemptions may apply)
