RN Care Manager: Expert Care Coordination & Discharge

Piedmont Atlanta Hospital is seeking a Care Manager to coordinate patient care across settings and drive timely, safe discharges. You will lead progression of care and discharge planning for a defined caseload, ensuring optimal patient outcomes.

Requirements include graduation from a nursing program and Georgia RN licensure, with 2 years in acute/post-acute settings. A Master’s degree and Case Management certification are preferred.
