RN Care Manager - Compassionate Care

Working remotely, the full-time RN Care Manager - Compassionate Care will manage the healthcare journey of patients with kidney disease, focusing on personalized care, advance care planning, and collaboration with interdisciplinary teams to ensure optimal patient outcomes. Key responsibilities Build empathetic relationships with patients and families while providing education on diagnosis and treatment options Conduct advance care planning discussions to assess patient needs and develop collaborative care plans Coordinate care across multiple providers and disciplines, ensuring accurate documentation and communication of the plan of care Required qualifications Active RN license in good standing 3+ years of clinical practice in an intensive care unit or hospice/palliative care role Experience with goals of care discussions and end-of-life care decisions Intermediate skills with MS Office Suite, including Outlook and Teams Able to work effectively in a remote environment with required internet speeds
