Care Coordinator

The Care Coordinator is responsible for providing comprehensive care management for assigned patients during an episode of illness or treatment. This role involves collaboration with patients, their families, physicians, the patient care team, and payors to ensure efficient and effective healthcare delivery.

Key Responsibilities:

• Utilize advanced clinical skills to facilitate care that includes assessment, planning, intervention, and evaluation principles of the nursing process.
• Promote and evaluate the effective utilization of resources through current clinical knowledge and awareness of community services.
• Take on a leadership role within the patient care team to achieve optimal clinical and resource outcomes.
• Evaluate and analyze quality and utilization metrics, including admission/discharge criteria, observation stays, readmissions within 30 days, and Medicare-related criteria.
• Participate in ongoing outlier review processes.
• Advocate for patients and their families throughout the entire episode of care.
• Maintain availability as a resource for patients and families to facilitate communication with providers and monitor services rendered.
• Remain involved until the patient is discharged or appropriately transferred to an acute care setting or alternate care level.
• Initiate appropriate clinical pathways based on admission diagnoses and serve as a resource for utilization management purposes.

Minimum Requirements:

• At least one year of recent case management experience in an acute care setting.
• Current California Licensure.
• Current CPR certification.

This position may involve assignments in emergency department settings to ensure efficient and cost-effective care for assigned cases.
