RN Care Manager – Case Management (2pm–10:30pm)

Baylor Scott & White Health is seeking an experienced Case Manager in Fort Worth, TX. You will oversee patient care using guidelines to determine benefits and resolve service necessity issues.

Multiple roles will be adopted to promote optimal outcomes and efficient care coordination across the healthcare team. Qualifications include an Associate's degree, RN license, and at least two years of hands-on experience in care coordination.
